Australian Government’s Murray Primary Health Network selects HealthBeats® for Remote Patient Monitoring

Murray Primary Health Network (Murray PHN) has selected HealthBeats® Remote Vitals Monitoring platform to support patients with chronic diseases. HealthBeats® provides end-to-end solution including all medical devices, software platform and support services to participating health services and their patients.

 

Murray PHN is an Australian government initiative to hear, understand, and address the health needs of our communities by engaging key partners to deliver targeted actions that increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes and improve the coordination of care to ensure patients receive the right care in the right place at the right time.

 

The region spans from Mildura in the North West, to Woodend in the south, across to Seymour and up to Albury - an area of almost 100,000 square kilometres divided into 4 regions (Central Victoria, Goulburn Valley, North East and North West Victoria) and includes 22 Local Government Areas (LGAs). Murray PHN has 55 hospital services, 214 general practices and looks after approximately 370,000 chronic patients.
