CareJourney: A New Structured Support Scheme for Caregivers Announced at Caregiving Fest 2025

At Caregiving Fest 2025, the Caregiving Welfare Association (CWA) introduced CareJourney, a new initiative offering caregivers coordinated support, training and direct access to community partners without having to re-explain their needs. The programme supports new, active and former caregivers with resources ranging from step-by-step guides to emotional support groups and mentoring opportunities. CareJourney was launched alongside CWA's home-based monitoring programme, both aimed at providing immediate, scalable and sustainable solutions for caregivers and seniors.

Advancing Care Through Evidence and Technology
In his opening address, Dr Tan Jit Seng, President of CWA, unveiled a white paper detailing the outcomes of the organisation’s home-based monitoring pilot in August 2024. The system tracks seniors’ wellbeing through sensors that monitor vital signs and activity and will trigger real-time alerts. These alerts will be assessed by the CWA care team, who verify and filter false alarms before activating appropriate responses involving trained volunteers and community responders.

While the pilot faced early challenges with technology adoption and adherence among seniors, the pilot services as a proof-of-concept for the organisation's goal of supporting caregivers and seniors sustainably into the future. “Our focus this year is on solutions that are immediate, scalable, and grounded in evidence. Through CareJourney and Home-based Monitoring, we aim to ensure that seniors and their caregivers never feel alone in their journey.” said Mr Steven Yeo, Executive Director of CWA.
